How to report a work comp claim
All employee work-related injuries or illnesses should be reported immediately to ensure prompt medical attention and the seamless service you can count on throughout the work comp claims process.
What to do during a medical emergency
- Call 911 for medical instruction and emergency access to care.
- Individuals on scene follow an employer’s incident reporting protocol.
What to do during a non-emergency injury
If the injury is not life-threatening:
- Direct the employee to your authorized medical provider, or
- Call CorVel Advocacy 24/7 Triage Telemedicine for nurse guidance at 844-842-2674
- Call 855-810-9199 if you're located in Idaho, Montana, Nevada, New Mexico or Oregon
- A registered nurse will help determine appropriate next steps
- Important: The triage call is for medical guidance only and does not report a claim to UFG. If an injury results in first-aid or self-care treatment only, it is not necessary to report the claim to UFG
- Be sure to complete your company’s incident reporting requirements
In all cases, employers must follow these steps:
- An employer will complete the state specific First Report of Injury (FROI)
- Submit it to UFG Insurance by one of the following methods:
- UFG Email: newclaims@unitedfiregroup.com.
- Send the FROI directly to their UFG Agent to submit to UFG.
- UFG Fax: 888-514-9190.
